Position Overview
As a Licensed Practical Nurse (LPN) specializing in hospice care, you will be instrumental in delivering compassionate nursing services to patients in their homes during weekends.
Sign-on Bonus: A $1,500 bonus is available for candidates with relevant experience in HomeCare Homebase.
Work Schedule: This is a full-time role requiring 80 hours per pay period with variable shifts.

Since 1978, we have dedicated ourselves to ensuring that our patients experience comfort, dignity, and independence in their own homes. Each year, we care for over 6,300 patients through our comprehensive HomeCare and Hospice programs.
Role Summary:
In this position, you will provide nursing care to a designated group of homebound patients under the guidance of a Registered Nurse. Your responsibilities will include collaborating with other healthcare professionals and reporting directly to the assigned Clinical Supervisor.
Key Responsibilities:
- Conduct and document thorough assessments including cardiovascular, pulmonary, neurological, gastrointestinal, skin, intake/output, and pain evaluations.
- Administer treatment procedures such as the insertion and management of feeding tubes, and provide education to patients and caregivers regarding tube care.
- Perform intravenous procedures, including the insertion of peripheral IV lines and maintaining infusion rates (IV Certification required).
- Deliver specialized care including colostomy and tracheostomy management, as well as suctioning.
- Administer medications in accordance with HomeCare policies, ensuring accurate documentation and adherence to safety protocols.
- Maintain compliance with Medicare guidelines and ensure that care plans are followed and adjusted as necessary.
- Collaborate with the healthcare team to enhance patient care outcomes and assist in implementing the nursing care plan.
- Adhere to infection control standards and proper waste disposal methods as per Covenant HomeCare policies.
- Manage patient schedules and communicate arrival times effectively.
- Participate in ongoing education and quality improvement initiatives.
Qualifications:
Education: No specific educational requirements; however, qualifications must meet the standards for licensure.
Experience: A minimum of one year of professional experience in a hospital or home health setting is required, along with successful completion of an LPN advanced skills course.
Licensure: Candidates must possess and maintain a valid Tennessee LPN license or Licensed Vocational Nurse certification. CPR certification is mandatory, and a valid Tennessee driver's license with state-required insurance coverage is necessary.
Additional Information: Local travel is required for this position. Adherence to policies, procedures, and safety standards is essential.
