Job Summary

The MFM Complex RN Care Manager plays a vital role in providing patient-centered care to individuals with high-risk maternity needs. This position requires collaboration with the interprofessional team to develop, execute, and evaluate patient care plans. The ideal candidate will possess strong communication skills, the ability to make informed decisions, and a commitment to delivering high-quality care.

Key Responsibilities
  • Assess patient records to identify high-risk maternity needs and develop individualized care plans.
  • Collaborate with the interprofessional team to provide comprehensive care, including telephone triage, patient education, and medication management.
  • Coordinate patient services, including scheduling and referrals, to ensure seamless care transitions.
  • Provide education and support to patients and their families, addressing their unique needs and concerns.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Associate Degree in Nursing or equivalent required.
  • Current North Carolina nursing license or Board eligibility required.
  • Current CPR certification required.
  • 2+ years of relevant experience in OB/GYN or high-risk maternity care preferred.
Preferred Qualifications
  • 2+ years of experience in an OB/GYN setting.
  • Spanish speaking skills preferred.
Work Environment

This role requires regular attendance on-site, with typical business hours Monday-Friday, 8:00 am-5:00 pm. Weekend, holiday, or evening coverage may be required. The ideal candidate will be flexible and able to adapt to changing work assignments and priorities.
