Key Responsibilities
  • Perform general outreach, engagement, and follow-up with members to ensure they receive the care and support they need.
  • Coordinate services and appointments, including arranging transportation and providing wellness reminders.
  • Engage in health promotion activities and knowledge sharing to promote healthy behaviors and outcomes.
  • Share information with the care manager and other team members to ensure a comprehensive understanding of each individual's circumstances.
  • Provide and track referrals, and assist individuals in obtaining and maintaining community-based resources and social support services.
  • Participate in case conferences and support the care manager in assessing and addressing unmet health-related resource needs.
Requirements
  • Valid driver's license and reliable transportation.
  • High school diploma or equivalent, and at least 18 years of age.
  • Meet one of the following requirements: lived experience with an I/DD or TBI, lived experience with a behavioral health condition and certified peer support specialist, or two years of experience providing care and navigating the Medicaid delivery system.
