Key Responsibilities- Collaborate with Patients and Families: Work closely with patients, families, and staff across all disciplines and departments to provide comprehensive care coordination and cost management for patients with chronic illness.
- Develop and Communicate Care Plans: Assess patient and primary caregiver needs, develop and communicate comprehensive care plans based on evidence-based best practices, and ensure seamless transitions between care settings.
- Monitor Patient Conditions: Regularly monitor patient conditions, identify potential issues, and take proactive steps to optimize patient outcomes.
- Coordinate Care Transitions: Manage and coordinate all transitions between care settings, providers, and services to ensure continuity of care.
- Provide Education and Support: Educate and support patients and families on disease management, self-care, and community resources.
- Facilitate Access to Community Services: Facilitate patient access to community services, resources, and support groups to promote optimal health outcomes.
- Provide Direct Nursing Care: Provide direct nursing care to patients, including medication management, wound care, and other interventions as needed.
- Patient Triage: Triage patient needs, prioritize care, and ensure timely interventions to prevent hospitalizations and readmissions.
- Optimize Patient Outcomes: Proactively manage and follow-up with patients to ensure care plans are in place, and patient outcomes are optimized.
- Document Patient Care: Complete accurate and timely documentation of patient care, including progress notes, care plans, and other relevant information.
- Participate in Quality Initiatives: Participate in quality improvement initiatives, patient safety programs, and other efforts to promote excellence in patient care.
- Education: Graduation from an accredited school of nursing.
- Experience: Minimum of 2 years of clinical nursing experience.
- Certifications: Current license to practice nursing in the state of Texas and current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross.
