RN Care Coordinator

At UnitedHealthcare, we're simplifying the healthcare experience, creating healthier communities, and removing barriers to quality care. We're seeking a skilled RN Care Coordinator to join our team, working closely with nursing facilities to ensure our members receive the best possible care.
Key Responsibilities:
- Assess, plan, and implement care strategies tailored to individual member needs
- Identify and initiate referrals for member transition to community resources and provide assistance with social service programs
- Manage care plans throughout the continuum of care as a single point of contact
- Communicate with stakeholders to ensure quality coordinated care and services are provided
- Advocate for members and families to ensure their needs and choices are represented and supported
Requirements:
- Current, unrestricted RN license in the state of Texas
- 2+ years of experience working in a community health setting or healthcare role
- Intermediate level of computer proficiency and ability to use multiple web applications
- Reliable transportation and ability to travel in the assigned region to visit members in their homes and other settings
- Access to a designated quiet workspace with the ability to secure Protected Health Information (PHI)
Preferred Qualifications:
- 1+ years of experience with long-term care services and support, Medicaid, or Medicare
- Ability to create, edit, save, and send documents, spreadsheets, and emails
