Position Overview

Role Objective:

As the Client Intake Coordinator, you will evaluate clients for suitable entry into treatment programs by completing various assessment forms, including Biopsychosocial, ANSA, SOGA, and health risk screenings. You will create an Initial Treatment Plan for clients and ensure all necessary documentation is accurately completed. You will facilitate the transition of clients to primary clinicians and case managers as required and collaborate with external referral sources to meet clients' service needs. The Client Intake Coordinator will play a vital role in fulfilling the mission of The Salvation Army.

Key Responsibilities:
  • Conduct intake and admission processes, gathering essential demographic and background information, completing SOGA and TCUDS, and assessing clients' needs, strengths, and preferences.
  • Perform assessments for Withdrawal Management clients within 48 hours of admission, provided the client is medically stable.
  • Enroll and re-enroll eligible clients in funding programs by completing necessary enrollment documentation.
  • Develop an Initial Treatment Plan to commence client treatment.
  • Complete the Biopsychosocial assessment with the client.
  • Prepare an action sheet regarding the client's level of care placement.
  • Draft a clinical summary detailing the client's history and presenting issues.
  • Utilize DSM V criteria to accurately diagnose clients based on interview findings.
  • Participate in weekly multi-disciplinary team meetings to evaluate treatment levels and diagnoses.
  • Maintain client confidentiality in accordance with federal regulations and organizational standards.
  • Attend scheduled staff meetings, departmental meetings, seminars, and training programs.
  • Ensure all certifications are current by meeting necessary requirements.
  • Input all required client information into the CSM computer system.
  • Perform additional tasks as assigned by leadership.
Minimum Qualifications:

Education: A Master's Degree or a Licensing Track (License Mental Health Counselor LMHC or License Clinical Addiction Counselor LCAC), LCSW (Licensed Clinical Social Worker).

Background Checks: This position necessitates a background check, and findings may disqualify an individual.

Experience: A minimum of two years of office experience.

Certifications: Must be pursuing or currently possess CADAC certification. A valid driver's license is required, along with compliance with The Salvation Army's driver qualification standards; Safe From Harm training must be completed within the first 90 days of employment.

Skills and Abilities:
  • Proficient in speaking, writing, and understanding English for effective communication with leadership, field personnel, and clients.
  • Computer literate with Microsoft products and capable of learning electronic reporting systems.
  • If in recovery, must demonstrate a minimum of two years of sobriety, including emotional and social stability.
Supervisory Responsibilities: None
