Job Summary

We are seeking a compassionate and skilled Palliative Care Social Worker to join our team. As a Palliative Care Social Worker, you will provide psychosocial expertise and attention to the social determinants of health, addressing complex issues of patients and families in the palliative care setting.

Key Responsibilities
  • Collaborate with the interdisciplinary palliative care team members, communicating clearly and effectively as the psychosocial expert on the team to achieve high-quality palliative care for all patients.
  • Assess patients' ability to complete advance care planning documents, providing education, support, and resources for completing advance care planning documents.
  • Conduct psychosocial assessments for patients and family members, utilizing clinical skills to identify strengths and barriers related to social determinants of health.
  • Provide anticipatory grief support to terminal patients & family members experiencing social, emotional, behavioral & psychiatric problems during a transition to end-of-life.
  • Promote healthy discussion between the patient, family, and medical team, facilitating understanding of the patient's medical condition and treatment options.
  • Document all psychosocial interventions, capturing the patients' story of illness, cultural perspective, socio-economic status, and religious beliefs.
Requirements
  • Master's degree in Social Work
  • 3 years of job-related experience
  • Advanced Certified Hospice and Palliative Nurse (ACHPN) certification
  • Future - Advanced Palliative Hospice Social Worker - Certified within 36 months
  • Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Social Worker (LCSW), Licensed Marital and Family Therapist (LMFT) or Licensed Professional Counselor (LPC) licensure
Work Environment

Work in a constant state of alertness and safe manner. Frequent reaching, sitting, standing, and walking. Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more).
