About the Role

SSM Health is seeking a highly skilled and compassionate Palliative Care Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals and their families in the palliative care setting.

Key Responsibilities
  • Collaborate with the interdisciplinary palliative care team to provide comprehensive patient care and support.
  • Assess patients' psychosocial needs and develop personalized care plans to address complex issues.
  • Provide education and support to patients and families on advance care planning, including health care proxies, living wills, and other accepted forms of advanced care planning documents.
  • Conduct psychosocial assessments and provide anticipatory grief support to patients and families experiencing social, emotional, behavioral, and psychiatric problems.
  • Promote healthy discussions between patients, families, and medical teams to facilitate understanding of medical conditions and treatment options.
  • Document all psychosocial interventions, including assessments and care plans, to capture patients' stories, cultural perspectives, socioeconomic status, and religious beliefs.
  • Deliver education on palliative care social work to fellows, residents, medical students, and other learners.
  • Coordinate with case management, risk management, legal departments, and ethics committees to advocate for best possible outcomes for patients.
Requirements
  • Master's degree in Social Work.
  • At least three years of job-related experience.
  • Advanced Certified Hospice and Palliative Nurse (ACHPN) certification or future certification within 36 months.
  • Licensed Clinical Social Worker (LCSW), Licensed Marital and Family Therapist (LMFT), or other relevant licenses.
