About the Role:
SSM Health is seeking a compassionate and skilled Palliative Care Social Worker to join our team. As a Palliative Care Social Worker, you will play a vital role in providing psychosocial expertise and support to patients and families in the palliative care setting.
Key Responsibilities:
- Collaborate with the interdisciplinary palliative care team to provide comprehensive care to patients and families.
- Assess patients' psychosocial needs and develop individualized plans to address these needs.
- Provide education and support to patients and families on advance care planning, including health care proxy, living will, and other accepted forms of advanced care planning documents.
- Conduct psychosocial assessments and provide anticipatory grief support to patients and families experiencing social, emotional, behavioral, and psychiatric problems during a transition to end-of-life.
- Promote healthy discussion between patients, families, and medical teams to facilitate understanding of patients' medical conditions and treatment options.
- Document all psychosocial interventions, including assessments and plans of care, to capture patients' stories of illness and how their cultural perspectives, socioeconomic status, and religious beliefs impact their well-being.
Requirements:
- Master's degree in Social Work.
- At least three years of job-related experience.
- Advanced Certified Hospice and Palliative Nurse (ACHPN) certification or future certification within 36 months.
- Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Social Worker (LCSW), Licensed Marital and Family Therapist (LMFT), or Licensed Professional Counselor (LPC) licensure in the state of employment.
