About the Role

We are seeking a highly skilled and compassionate Social Worker to join our Palliative Care team at SSM Health. As a Palliative Care Social Worker, you will play a vital role in providing psychosocial expertise and support to patients and their families in our palliative care setting.

Key Responsibilities
  • Collaborate with the interdisciplinary palliative care team to provide high-quality care to patients and their families.
  • Assess patients' ability to complete advance care planning documents and provide education, support, and resources to facilitate completion.
  • Conduct psychosocial assessments to identify strengths and barriers related to social determinants of health.
  • Provide anticipatory grief support to patients and their families experiencing social, emotional, behavioral, and psychiatric problems.
  • Promote healthy discussion between patients, families, and medical teams to facilitate understanding of medical conditions and treatment options.
  • Document all psychosocial interventions, including assessments and plans of care.
  • Deliver education on palliative care social work to healthcare professionals and learners.
  • Coordinate with case management, risk management, legal department, and ethics committee to advocate for best possible outcomes for patients.
Requirements
  • Master's degree in Social Work.
  • At least 3 years of job-related experience.
  • Advanced Certified Hospice and Palliative Nurse (ACHPN) certification or future certification within 36 months.
  • Licensed Clinical Social Worker (LCSW), Licensed Marital and Family Therapist (LMFT), or Licensed Professional Counselor (LPC) licensure in the state of employment.
