Optum Clinical Practice Performance Coordinator Job Description

As a Clinical Practice Performance Coordinator, you'll play a critical role in driving clinical relationships and engagement with physician practices, members, and pharmacies. Your goal will be to improve health, well-being, quality, and practice performance while reducing medical costs.

Key Responsibilities:
  • Develop and implement comprehensive, provider-specific plans to increase physician practice performance and reduce readmissions.
  • Collaborate with internal teams, including Network contract ACO managers, Health Care Economics and Analytics, Medical Directors, Reporting, and Health Plan market leaders, to drive clinical relationships and engagement.
  • Utilize data analytics, technology, and workflow changes to identify performance improvement opportunities and proactively address them.
  • Facilitate effective education and reporting to support clinical practice performance improvement.
  • Work with members with complex and/or chronic conditions to develop effective super utilizer engagement strategies.
Requirements:
  • High School Diploma/GED
  • 2+ years of experience as a pharmacy technician
  • 2+ years of telephonic customer service experience
  • 1+ year of healthcare background with medical terminology familiarity of clinical issues
  • 1+ year of working experience with ICD-9/10 and CPT Codes
  • 1+ year of working experience with Microsoft Tools: Word and Outlook
  • Ability to travel 60% of the time for business meetings and 40% remote work
  • Pharmacy Tech License
Preferred Qualifications:
  • Experience working in a physician, provider, and/or medical office
  • Medical Assistant
  • LPN
  • EMR and HEDIS knowledge and experience
