Job Summary

Varco Home Health and Hospice is seeking a skilled Home Health Nurse to join our team. As a Home Health Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Conduct Comprehensive Patient Assessments

Conduct thorough in-home health assessments to evaluate patients' vital signs, medication adherence, and overall health status.

  • Develop Personalized Care Plans

Develop individualized care plans in collaboration with the healthcare team, meeting patients' needs and goals.

  • Administer Medications and Treatments

Administer medications, treatments, wound care, and medical procedures following prescribed protocols and care plans.

  • Monitor and Document Patient Progress

Monitor and document patient condition changes, promptly communicating updates to the healthcare team.

  • Provide Education and Support

Educate patients and their families on disease management, medication adherence, and lifestyle adjustments to promote wellness.

  • Collaborate with Healthcare Professionals

Collaborate with various healthcare professionals to ensure a holistic approach to patient care, including physical therapists, occupational therapists, and social workers.

Requirements
  • Current LVN License

Current LVN license in the state of Texas.

  • Home Health Experience

Proven experience as an LVN, preferably in a home health setting.

  • Clinical Assessment and Patient Care Skills

Proficiency in clinical assessment and patient care.

  • Strong Communication and Interpersonal Skills

Strong communication and interpersonal skills.

  • Ability to Work Independently

Ability to work independently and as part of a team.

  • Valid Driver's License

Valid driver's license and reliable transportation.
