Key Responsibilities:- Oversee a team of 37 FTEs, working closely with the Home Health Supervisor to ensure seamless patient care.
- Assist with patient care, providing regular support and guidance to the team.
- Work Monday through Friday, with occasional weekend call as needed.
- Ensure compliance with local, state, and federal regulations, as well as company-established quality assurance standards.
- Manage a budgeting and accounting system to promote effective operation of the Home Health Agency.
- Coordinate patient care plans, ensuring timely and effective delivery of services.
- Assign staff to meet patient needs, providing orientation and support as necessary.
- Monitor patient care plans, ensuring execution as written.
- Facilitate communication between staff, patients, and physicians to optimize care delivery.
- Conduct regular team conferences to evaluate and revise patient care plans.
- Perform regular audits to ensure compliance with regulations and company standards.
- Participate in orientation of new staff and coordinate continuing education and staff development activities.
- Review clinical documentation for compliance with regulations and quality of care.
- Assist with implementation of the agency's Quality Improvement Plan and Medical Review process.
- 5 years of current Home Health RN experience, including at least 6 months of supervisor experience.
- Case manager experience preferred.
- BSN degree.
- AHA or ARC BLS Certification.
- Covid Vaccine or eligible exemption.
- TX RN license or active multi-state RN license.
