Job Summary:

The Nurse Manager - Home Health will oversee a team of healthcare professionals providing high-quality patient care in the comfort of their own homes. This leadership position requires a seasoned nurse with experience in home health care and a passion for delivering exceptional patient outcomes.

Key Responsibilities:
  • Oversee a team of 37 FTEs, working closely with the Home Health Supervisor to ensure seamless patient care.
  • Assist with patient care, providing regular support and guidance to the team.
  • Work Monday through Friday, with occasional weekend call as needed.
  • Ensure compliance with local, state, and federal regulations, as well as company-established quality assurance standards.
  • Manage a budgeting and accounting system to promote effective operation of the Home Health Agency.
  • Coordinate patient care plans, ensuring timely and effective delivery of services.
  • Assign staff to meet patient needs, providing orientation and support as necessary.
  • Monitor patient care plans, ensuring execution as written.
  • Facilitate communication between staff, patients, and physicians to optimize care delivery.
  • Conduct regular team conferences to evaluate and revise patient care plans.
  • Perform regular audits to ensure compliance with regulations and company standards.
  • Participate in orientation of new staff and coordinate continuing education and staff development activities.
  • Review clinical documentation for compliance with regulations and quality of care.
  • Assist with implementation of the agency's Quality Improvement Plan and Medical Review process.
Requirements:
  • 5 years of current Home Health RN experience, including at least 6 months of supervisor experience.
  • Case manager experience preferred.
  • BSN degree.
  • AHA or ARC BLS Certification.
  • Covid Vaccine or eligible exemption.
  • TX RN license or active multi-state RN license.
