Job Opportunity

Your Tailor Made Senior Service is seeking a skilled Registered Nurse to provide high-quality care to our patients in the comfort of their own homes.

Responsibilities
  • Deliver Quality Care: Provide comprehensive nursing care to patients in accordance with their individualized plans of care.
  • Assess and Evaluate: Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Collaborate with the Care Team: Work closely with the interdisciplinary team to ensure seamless care coordination and communication.
  • Document Patient Progress: Accurately document patient interactions, assessments, and care provided to ensure continuity of care.
  • Manage Patient Services: Oversee the delivery of patient services, including nursing treatments and activities, to meet individual patient needs.
  • Review and Revise Care Plans: Regularly review patient progress and revise care plans as needed to ensure optimal outcomes.
  • Meet Productivity Expectations: Maintain a high level of productivity, with a weekly expectation of 30 units of productivity.
Requirements
  • OASIS/Medicare Certified Visits Experience: Possess experience with OASIS/Medicare certified visits.
  • Electronic Charting Experience: Have experience with electronic charting systems.
  • Developed Computer Skills: Possess strong computer skills and the ability to learn new systems quickly.
  • Minimum of 1 Year of Recent Home Health Nursing Experience: Have a minimum of 1 year of recent experience in home health nursing.
  • On-Call Requirements: Be available for on-call shifts as required by contracts.
  • Driver's License and Vehicle: Possess a valid driver's license and a reliable vehicle with current auto insurance.
  • American Heart Association BLS: Hold current American Heart Association BLS certification.
  • Valid U.S. Driver's License: Possess a valid U.S. driver's license.
  • Problem-Solving Skills: Possess strong problem-solving skills and the ability to think critically.
  • Excellent Communication Skills: Have excellent communication skills and the ability to work effectively with patients, families, and healthcare teams.
  • Positive, Professional Attitude: Possess a positive, professional attitude and a commitment to delivering exceptional patient care.
