Job Summary

We are seeking a skilled Physical Therapist to provide high-quality physical therapy services to patients in their homes. As a Physical Therapist with our team, you will be responsible for assessing and evaluating patients' therapeutic, rehabilitative, and functional status, and developing a comprehensive plan of care to achieve their goals.

Key Responsibilities:
  • Provide physical therapy services to patients according to a written physician's plan of care, focusing on improving mobility, strength, and range of motion.
  • Assess and evaluate patients' muscle strength, mobility, gait, and range of motion, identifying areas for improvement and developing targeted treatment plans.
  • Direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances, and provide education on proper exercise techniques and home safety.
  • Determine priority needs for physical therapy, ensuring timely and effective interventions to promote optimal patient outcomes.
  • Report to physicians on patients' reactions to treatment or changes in condition, providing regular updates and recommendations for further care.
  • Initiate physical therapy programs, instruct other personnel and/or family/caregivers in certain phases of physical therapy, and may train patients in the use of prosthetic devices.
  • Identify patients' and families'/caregivers' needs for other home health services and refer as necessary, ensuring a comprehensive approach to patient care.
  • Prepare and submit clinical and progress summaries based on the attainment of goals, providing regular updates to patients, families, and healthcare teams.
  • Participate in discharge planning for patients, ensuring a smooth transition to post-acute care or community-based services.
  • Provide physical therapy consultation to families/caregivers when indicated, offering guidance and support to promote optimal patient outcomes.
  • Provide in-service education programs for nursing organization personnel as needed, sharing expertise and best practices in physical therapy.
  • Participate in peer consultation process, engaging in regular discussions and feedback with colleagues to enhance clinical practice and patient care.
  • Supervise Physical Therapy Assistants according to organization policy and state regulations, ensuring high-quality care and adherence to standards.
  • When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks, ensuring alignment with patient needs and goals.
Requirements:
  • Possess a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
  • Licensed to practice as a physical therapist within the state of Texas.
  • Two years of appropriate experience as a physical therapist, with community/home health experience preferred.
  • Demonstrate good verbal and written communication and organization skills, with the ability to work effectively in a fast-paced environment.
  • Possess and maintain current Heart Association-approved CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order.
Medical Requirements:
  • Current Tuberculosis (TB) Test or Chest X-ray (within the last six months).
  • Titers for: Varicella (Chickenpox), MMR (Measles, Mumps, Rubella), and Hepatitis B.
  • Proof of Vaccination for Tdap (Tetanus, Diphtheria, Pertussis).
  • Proof of Vaccination for Flu (Influenza).
  • Proof of Physical exam (within the last six months).
