Care Coach Connect RN Case Manager Midland TX and Surrounding Areas

At Optum, we're committed to delivering high-quality care to our patients. As a Care Coach Connect RN Case Manager, you'll play a vital role in coordinating care and providing ongoing nursing assessments to patients and their families.
Key Responsibilities:- Conduct medical home visits and urgent/acute visits to patients, performing hands-on exams and assessments.
- Develop and implement individualized plans of care, recommending revisions as necessary.
- Collaborate with interdisciplinary teams to ensure seamless care coordination.
- Provide education and support to patients and families on disease management, pain and symptom control, and end-of-life care.
- Initiate preventive and rehabilitative nursing procedures as needed.
- Registered Nurse with a current license to practice in the state of employment.
- 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
- Excellent nursing skills, computer proficiency, and ability to interact effectively with individuals and teams.
- Reliable transportation for daily travel to various locations, up to 80% of the time.
- Hospice and Palliative Care Nursing certification.
- 2+ years of hospice experience.
- Experience with home visits, advanced care planning, and community-based nursing.
