Care Coach Connect RN Case Manager Midland TX and Surrounding Areas

As a Care Coach Connect RN Case Manager, you'll play a vital role in coordinating care and providing ongoing nursing assessments to patients and their families. Your expertise will help ensure that our patients receive the highest level of medical care, and you'll be part of a team that shares your passion for helping people achieve better health.
Key Responsibilities:- Conduct medical home visits and urgent/acute visits to assess patient needs and coordinate care with interdisciplinary team members.
- Provide ongoing assessments of patient physical, functional, psychosocial, and environmental needs, and implement individualized plans of care.
- Assess caregiver burnout and provide community resources as needed.
- Collaborate with providers to recommend policies, procedures, and standards that affect patient care.
- Document progress toward established goals and maintain accurate records in the electronic health record.
- Registered Nurse with a current license to practice in the state of employment.
- Current BLS certification or must obtain certification within 30 days of employment.
- 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
- Excellent nursing skills, computer proficiency, and ability to interact productively with individuals and teams.
- Reliable transportation for daily travel to various locations, up to 80% of the time.
- Hospice and Palliative Care Nursing certification.
- 2+ years of hospice experience.
- Experience with home visits, advanced care planning, and community-based nursing.
- Bilingual (English/Spanish) language proficiency.
