Care Coach Connect RN Case Manager Midland TX and Surrounding Areas

We are seeking a highly skilled RN Case Manager to join our Care Coach Connect team in Midland, TX. As a RN Case Manager, you will play a critical role in coordinating care and providing ongoing nursing assessments for patients and their families.
Key Responsibilities:- Conduct medical home visits and urgent/acute visits on established Care Coach Connect patients
- Provide ongoing assessments of patients' physical, functional, psychosocial, and environmental needs
- Assess for caregiver burnout and provide community resources as needed
- Implement individualized plans of care and recommend revisions as necessary
- Ability to perform procedures such as wound care, phlebotomy, and injections
- Conduct advanced care discussions with patients and their families and document their wishes in the electronic medical record
- Consult and educate patients, families, and caregivers on disease processes, pain and symptom management, end-of-life care, and ethical concerns
- Initiate preventive and rehabilitative nursing procedures as needed
- Ensure continual assessment of patient and family needs from admission to Care Coach Connect throughout the course of care
- Provide ongoing evaluation of patient and family/caregiver response to care and recommend alterations to the plan when necessary
- Registered Nurse with a current license to practice in the state of employment
- Current BLS certification or must obtain certification within 30 days of employment
- 3+ years of experience in a physician's office, clinical, hospice, or hospital setting
- Excellent nursing skills and ability to interact productively with individuals and multidisciplinary teams
- Reliable transportation for daily travel to various locations as assigned, up to 80% of the time
- Hospice and Palliative Care Nursing certification
- 2+ years of hospice experience
- Experience with home visits, advanced care planning, and community-based nursing
- Bilingual (English/Spanish) language proficiency
- Ability to lift up to 100 pounds
- Ability to push or pull heavy objects using up to pounds of force
- Ability to sit for extended periods of time
- Ability to stand for extended periods of time
- Ability to use fine motor skills to operate office equipment and/or machinery
- Ability to properly drive and operate a company vehicle
