Care Coach Connect RN Case Manager

Your expertise will help ensure that patients receive the best possible care, while also promoting healthy outcomes and reducing healthcare costs.

Key Responsibilities:
  • Conduct medical home visits and urgent/acute visits to assess patient needs and develop individualized plans of care
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions
  • Provide education and support to patients and their families on disease management, pain and symptom management, and end-of-life care
  • Initiate preventive and rehabilitative nursing procedures as needed
  • Document patient progress and outcomes in the electronic health record
Requirements:
  • Registered Nurse with a current license to practice in the state of employment
  • 3+ years of experience in a physician's office, clinical, hospice, or hospital setting
  • Excellent nursing skills and ability to interact productively with individuals and teams
  • Reliable transportation for daily travel to various locations
Preferred Qualifications:
  • Hospice and Palliative Care Nursing certification
  • 2+ years of hospice experience
  • Experience with home visits and community-based nursing
