Job Summary

The Veterans Affairs, Veterans Health Administration is seeking a skilled Registered Nurse to join our Referral Coordination Team. As a key member of our team, you will play a vital role in ensuring timely access to care for our Veterans.

Responsibilities
  • Manage the care of Veterans throughout the consult process, serving as a liaison to internal and community providers.
  • Execute a streamlined approach to receiving, triaging, and directing timely, coordinated care.
  • Utilize nursing knowledge and systems theory to assess, organize, facilitate, and guide Veterans through their full range of care options.
  • Triangulate incoming referrals, discuss available care options, provide scheduling guidance, and serve as a resource within the service.
  • Collaborate with the Office of Integrated Veteran Care to triage Request for Services (RFS) and promote in-house service access.
  • Participate in activities and strategies to sustain an evidence-based practice culture.
Requirements
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • English Language Proficiency.
Preferred Experience
  • 3-5 years of Mental Health experience with both inpatient and outpatient Mental Health.
  • 1-2 years of Case Management experience.
Physical Requirements

This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, kneeling, sitting, bending, pulling, and pushing.
