About the Role

We are seeking a highly skilled and detail-oriented Care Transitions Coordinator to join our team at Beth Israel Deaconess Medical Center. As a key member of our care transitions team, you will play a critical role in ensuring seamless transitions for our patients from hospital to home or other post-acute care settings.

Key Responsibilities
  • Administrative Support: Provide administrative support to the RN Case Managers, including entering referrals, gathering clinical information, and verifying patient demographics.
  • Documentation and Communication: Prepare and distribute regulatory notices, complete required documentation, and communicate with patients, families, and healthcare providers as needed.
  • Post-Acute Referrals: Assist with securing post-acute transition services, including verifying insurance and identifying in-network providers.
  • Collaboration and Problem-Solving: Collaborate with RN Case Managers to facilitate timely discharge and identify and communicate any problems or issues that affect departmental goals and outcomes.
Requirements
  • Education: High School Diploma or GED; Associate's or Bachelor's Degree preferred.
  • Experience: Minimum 2 years of experience in healthcare, with strong computer skills and knowledge of medical terminology.
  • Certifications: BLS certification required.
