We are seeking a highly skilled and detail-oriented Care Transitions Coordinator to join our team at Beth Israel Deaconess Medical Center. As a key member of our care transitions team, you will play a critical role in ensuring seamless transitions for our patients from hospital to home or other post-acute care settings.
Key Responsibilities- Administrative Support: Provide administrative support to the RN Case Managers, including entering referrals, gathering clinical information, and verifying patient demographics.
- Discharge Planning: Assist with securing patient resources, monitoring post-acute referral status, and following up on screening determinations as necessary.
- Communication: Collaborate with RN Case Managers to facilitate timely discharge to the next level of care, and communicate effectively with patients, families, and healthcare providers.
- Documentation: Collect and input data into various databases, and provide documentation to patients, families, and caregivers under the direction of RN Case Managers.
- Problem-Solving: Identify and communicate problems or issues that affect departmental goals and outcomes, and perform routine tasks such as opening mail and providing accurate information to callers.
- Education: High School Diploma or GED required; Associate's or Bachelor's Degree preferred.
- Experience: Excellent computer skills, including proficiency with various computer systems; strong communication, interpersonal, and customer service skills; excellent assessment and problem-solving skills.
- Certifications: BLS certification required.
