Job Title: Care Coordinator

Muir Wood Adolescent & Family Services is seeking a compassionate and dedicated Care Coordinator to join our treatment team. As a Care Coordinator, you will play a vital role in providing structure, supervision, and direction to our clients, promoting accountability, and ensuring their safety and wellness.

Key Responsibilities:
  • Facilitate clients' daily activities in residential settings, including assistance with daily tasks, chores supervision, and meal service.
  • Conduct new client introductions, including search of all items prior to admittance and lock up of contraband and medications.
  • Observe and monitor clients' behavior, intervening as needed based on schedule, individual treatment plans, and house needs.
  • Provide one-to-one supervision of clients at risk for AMA, suicidal risk, eating disorders, etc., per directive of the Clinical Director.
  • Conduct Urinary Analysis screening and collection when directed by the Clinical Director, following appropriate procedures.
  • Transport clients in company vehicles to and from necessary appointments and off-property outings.
  • Coordinate milieu treatment with Counseling staff, ensuring effective communication and collaboration.
  • Supervise self-administration of client medications per physician orders and maintain training in medication dispensing.
  • Ensure physical plant safety and security by conducting regular shift checks, fire drills, and disaster drills.
  • Follow all emergency procedures, including paging protocol, transportation protocol, and other directives.
Requirements:
  • High school diploma or GED required.
  • Bachelor's degree in psychology, counseling, or sociology preferred.
  • Valid driver's license and eligibility for insurance coverage for driving company vehicles.
  • First Aid and CPR certification upon hire.
  • Prior work experience in behavioral health treatment settings with adolescents a plus.

Compensation:

$21.00 – $24.00/hour with flexibility based on education and experience
