Community Integration Specialist

At Inclusive Support Services NW LLC, we are seeking a highly skilled and compassionate Community Integration Specialist to join our team. This role is responsible for assisting individuals with disabilities in accessing and participating in community activities and opportunities.
Key Responsibilities:
- Conduct assessments to understand the needs, preferences, and abilities of clients.
- Develop individualized community integration plans tailored to each client's goals.
- Assist in the implementation of programs and activities that encourage socialization, skill development, and community involvement.
- Provide direct support to clients during community activities, ensuring their safety and well-being.
- Advocate for clients' rights and access to community resources and services.
- Teach and reinforce social, communication, and life skills to enhance clients' independence and confidence.
- Establish and maintain relationships with local businesses, service providers, and community organizations.
- Maintain accurate records of clients' progress, activities, and any incidents.
- Prepare reports and communicate regularly with supervisors and relevant stakeholders.
Requirements:
- High school education and Bachelor's Degree in related fields is preferred.
- Previous experience working with individuals with disabilities.
- Strong understanding of disability rights and inclusion principles.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
Working Conditions:
- The role may involve working evenings, weekends, and holidays to accommodate clients' schedules and community events.
- Physical activity may be required, including assisting clients with mobility and participating in various community activities.
