We are seeking a dedicated and compassionate Part-Time Registered Nurse to join our Clinical Care Delivery Team at Vynca. As a key member of our team, you will play a vital role in delivering high-quality, patient-centered care to individuals with complex social and clinical needs.
Key Responsibilities- Deliver nursing services aligned with individualized care plans, assessing treatment effectiveness and patient responses.
- Collaborate with interdisciplinary teams to ensure care goals are relevant and feasible.
- Record and communicate patient observations accurately within HIPAA-compliant systems.
- Facilitate case management and care coordination to uphold patient care plans.
- Educate patients and caregivers about VyncaCare services, palliative care, and disease management.
- Valid California Registered Nurse license.
- At least 2 years of clinical nursing experience in palliative care, hospice, or home health.
- Demonstrated proficiency in nursing practices, clinical judgment, and communication skills.
- Valid driver's license, insurance, and reliable vehicle.
- Proficiency in EMR, Google Suite, and other relevant technologies.
- Bachelor of Science in Nursing (BSN).
- CHPN certification or equivalent.
We offer a competitive compensation package, including a $10,000 incentive bonus and a salary range of $42-$52 per hour. The actual hourly rate will be based on your prior experience and skills.
