Job SummaryWe are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work with a variety of patients and provide comprehensive health assessments in the comfort of their own homes.
Responsibilities- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
- Provide health education and counseling to patients and their families
- Collaborate with physicians, case managers, and other healthcare professionals to ensure comprehensive care
- Document patient information and maintain accurate records
- Travel to patient locations, including homes, skilled facilities, and telehealth settings
- Master's degree in Nursing or related field
- Current RN and NP licensure in state of practice
- Board certification by AANP, ANCC, or AACCN
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent communication and interpersonal skills
