Job Overview:
Under the guidance of the Quality and Safety Director or Manager, the Patient Safety Coordinator plays a crucial role in the coordination and execution of comprehensive high reliability education and sustainment initiatives aimed at cultivating a culture of quality and safety among WellStar facility personnel, leadership, and medical staff.
This position is responsible for delivering educational programs at various levels, aligned with the WellStar Safety First initiative, and actively engages in the facility's peer safety and intervention strategies. The coordinator is involved in the safety event classification process, including weekly classification sessions, facilitates root cause analysis, conducts ongoing common cause analyses, and aids in the creation of annual patient safety reports and plans for departmental and organizational leadership.
Key Responsibilities:
Patient Safety Program Operations (Including Safety Event Management): Implementing and providing insights regarding the assessment of patient safety and clinical quality enhancement programs.
Assists in the development and delivery of training and educational resources for the Safety-First initiative, covering areas such as high reliability leadership techniques, error prevention tools, Fair and Just Culture principles, and Peer Reliability Coaches.
Reviews and revises policies and procedures as necessary.Delivers patient safety training across all levels.
Fosters strong relationships with key stakeholders and collaborates with System and Facility Safety Quality Staff to nurture a culture of safety and a learning environment.
Encourages a safety-oriented culture by participating in system-wide Culture of Safety Survey initiatives.Facilitates facility involvement in the Patient Safety Organization (PSO).
Supports and engages in the review, analysis, and reduction of quality measures, including but not limited to Patient Safety Indicators (PSI), Healthcare Associated Conditions (HACs), and other metrics as identified by the Centers for Medicare and Medicaid Services (CMS).
Analyzes facility safety event data to identify issues and trends for targeted improvement efforts.Ensures prompt investigation of patient safety concerns.
Facilitates data collection, analysis, and presentation of patient safety trends to leadership as required.
Participates in the management and tracking of safety events for the facility.
Assists in the implementation of patient safety initiatives and harm reduction strategies.
Supports WellStar's Root Cause Analysis process.
Guides unit/department leaders in the Apparent Cause Analysis (ACA) process as needed.
Actively engages in common cause analyses.
Performance Improvement:
Employs innovative design and systems thinking to enhance quality and safety metrics.
Assists in the formulation of measurement and patient safety improvement strategies.
Contributes to hospital performance improvement initiatives through problem assessment and educational efforts.
Regulatory Compliance:
Maintains an up-to-date understanding of relevant laws, regulations, statutes, and best practice literature impacting the Patient Safety Program.
Ensures compliance with regulatory standards.Financial Responsibility:
Demonstrates accountability in financial management, continuous quality improvement, institutional support, and customer service.
Education Requirements:
Bachelor's Degree in healthcare or a quality-related field.
Licenses and Certifications:
All certifications are required upon hire unless otherwise specified.
Basic Life Support-Preferred or BLS - Instructor-Preferred
Certified Professional in Healthcare Quality-Preferred
Additional Licenses and Certifications:
If an RN or other clinical discipline, must hold a valid license.
Experience Requirements:
Minimum of 1 year of experience in healthcare, nursing, or clinical practice is required, with experience in high reliability and safety design principles preferred.
Skills Required:
Strong organizational and interpersonal skills with the ability to work independently as well as collaboratively; Effective time management; Excellent written and verbal communication skills; High degree of professionalism, discretion, and confidentiality; Ability to build relationships and facilitate communication among stakeholders in both clinical and non-clinical environments; Proficient computer skills, particularly with Microsoft Office.
