Transplant Patient Care Specialist

The Transplant Patient Assistant plays a vital role in providing exceptional care to patients undergoing transplant procedures. This individual will work collaboratively with the healthcare team to coordinate patient care, ensuring a seamless experience for our patients and their families.
Key Responsibilities:- Collaborate with the healthcare team to develop and implement patient care plans
- Coordinate patient scheduling and appointments
- Provide emotional support and education to patients and their families
- Assist with administrative tasks, such as data entry and record-keeping
- Participate in performance improvement initiatives to enhance patient care and outcomes
- High School Diploma or equivalent required
- Graduate of a Medical Assistant program required
- Minimum two years of healthcare experience, preferably in a clinic setting
- Knowledge of patient scheduling and clinic processes
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bilingual or multilingual skills
- Basic Life Support (BLS) certification from the American Heart Association (AHA)
