Job Summary

As a Transplant Coordinator at Kaiser Permanente, you will play a critical role in providing exceptional care to patients undergoing renal transplantation. This is a challenging and rewarding position that requires strong clinical skills, excellent communication, and a passion for delivering high-quality patient care.

Key Responsibilities
  • Coordinate all aspects of transplant care, including pre-transplant evaluation, transplant surgery, and post-transplant follow-up.
  • Collaborate with a multidisciplinary team of healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide education and support to patients and their families throughout the transplant process.
  • Participate in quality improvement initiatives to enhance transplant care and patient satisfaction.
Requirements
  • Minimum 5 years of experience in critical care, ICU, emergency care, or transplant care services.
  • Bachelor's degree in nursing or a related field, or 4 years of experience in a directly related field.
  • Registered Nurse License (California) required.
Preferred Qualifications
  • Minimum 1 year of experience in clinical transplant coordination or case management.
  • Masters degree preferred.
