Job Summary:

The Transplant Patient Care Coordinator works under the supervision of the Kidney, Pancreas, and Transplant physicians and physician extenders in the Transplantation Institute, and reports to the Director.

Coordinates patient care activities, including obtaining new patient demographic, financial, and medical information, scheduling initial appointments, preparing patients for examination, obtaining authorizations for tests and procedures, and assisting with admissions into the hospital.

Triages calls and concerns from patients and healthcare professionals, participates in the department Performance Improvement program, and directs the flow of patients in the evaluation clinic.

Obtains medical records for clinic chart and enters data in the transplant database, utilizing interpersonal skills to maximize excellence in customer service, safe patient care, and professionalism.

Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population, providing services for both the LLUMC and LLUCH hospitals.

Performs other duties as needed.



Education and Experience:
High School Graduate or GED required. Completion of a Certified Medical Assistant or Licensed Vocational Nursing (LVN) program required. Minimum one year of previous clinical experience preferred.

Knowledge and Skills:

Able to write legibly, speak in English with professional quality, use computer, printer, and software programs necessary to the position, and operate patient care equipment necessary to the position.

Able to communicate in both English and Spanish preferred, relate positively, effectively, and professionally with others, be assertive and consistent in following policies, work calmly, and respond courteously when under pressure.

Able to communicate effectively in English in person, in writing, and on the telephone, follow directions, perform basic math functions, manage multiple assignments effectively, organize and prioritize workload, work well under pressure, recall information with accuracy, pay close attention to detail, and function within scope of practice.

Able to perform basic patient care activities using proper body mechanics and safety precautions.



Licensures and Certifications:
Medical Assistant Certification or LVN license required. Basic Life Support (BLS) certification required.
