We are seeking a skilled and compassionate Transformational Program Coordinator - Housing Support Specialist to join our team at East End Cooperative Ministry (EECM). In this role, you will play a vital part in changing the lives of individuals facing the impacts of poverty.

Job Description

As a Transformational Program Coordinator - Housing Support Specialist, you will be responsible for:



  2. Case Management: Complete intakes, assess client needs, and develop individualized case plans with long-term and short-term goals and objectives.
  3. Service Coordination: Coordinate and provide access to available alcohol and other drug or ancillary services, and make relevant referrals to agencies and service providers.
  4. Advocacy: Advocate for clients with agencies and service providers to minimize barriers to accessing necessary services, and follow up to ensure timely receipt of services.
  5. Stabilization: Assist in stabilizing clients' lives by determining the circumstances that led to their arrival at EECM.
  6. Coaching: Coach clients in job preparation, housing, available social services, and other areas as needed.
  7. Record Keeping: Maintain confidential files for each client, adhering to EECM's record retention policies.
  8. Policies and Procedures: Ensure clients adhere to shelter policies and procedures.
  9. Communication: Maintain regular communication with clients to provide ongoing positive reinforcement and ensure needed services are in place.


Secondary Responsibilities

In addition to the above duties, you will also be responsible for:



  2. Administrative Tasks: Perform administrative responsibilities such as greeting clients, guests, and visitors, and answering phones.
  3. Representation: Participate in activities or programs where representation has been requested.
  4. Other Duties: Perform other duties as assigned by management.


Requirements

To be successful in this role, you will need:



  2. Cultural Awareness: Cultural awareness training and experience working with people of diverse backgrounds, skills, and needs.
  3. Previous Experience: Previous experience providing services to individuals with mental health issues and/or experiencing homelessness, preferably in a residential setting.
  4. Clearances: Act 33 and 34 clearances are required.
  5. Certifications: CPR, first aid, and safety training certification may be obtained after hiring.
  6. Vehicle Requirements: Possess a reliable, insured vehicle and a current valid, regular Class A Pennsylvania driver's license with no accidents, points, or violations in the past 3 years.


Compensation and Benefits
