Program Overview:
The Cedarvale Supportive Housing Program offers accommodation for seniors who have faced homelessness and may be dealing with mental health and/or addiction challenges.

This program delivers comprehensive onsite psychosocial and primary care services, including intensive case management, harm reduction strategies, therapeutic and supportive counseling, round-the-clock staffing, assistance with daily living activities, nursing care, life skills training, social and recreational opportunities, volunteer engagement, and ongoing support.

Key Responsibilities:

Engage with program participants in person, providing emotional support and encouraging their active involvement in the program.

Deliver individualized support as specified in Coordinated Care plans, assisting social workers with transactional case management tasks such as documentation and navigating formal systems.

Facilitate the intake process through applicant selection, participation in interviews, managing intake, and following up with new participants.

Monitor the progress and well-being of participants, collaborating with the team to assess the effectiveness of Coordinated Care Plans and suggesting modifications as necessary.

Contribute to goal setting and the Coordinated Care planning process.

Design, implement, and oversee relevant individual and group programming for participants.

Assist participants in achieving greater independence within the program and the community.

Utilize the ADL/IADL tool regularly.

Participate in housing stabilization planning for participants by securing applications, monitoring waitlists, and liaising with housing assistance organizations.

Support skill development to help individuals perform daily living activities such as grooming, hygiene, shopping, banking, and informal safety training as outlined in Coordinated Care plans.

Encourage individuals to engage in activities that promote their independence and community participation, identifying services and activities that meet their needs while providing guidance and support.

Collaborate with other service providers and the wider community to establish and strengthen networks.

Recognize and assess potential aggressive behavior or crisis situations, developing strategies to address these incidents and informing the team and management as needed.

Execute delegated tasks as directed by healthcare professionals, including medication management in accordance with primary care instructions.

Arrange transportation for participants and accompany them to appointments and community activities when necessary.

Maintain accurate documentation and case notes efficiently.

Stay informed about internal fire regulations, emergency procedures, and ensure compliance with health and safety standards.

Contribute to program development and participate in program evaluations.

Qualifications:

Social Service Worker diploma or equivalent education.

Minimum of 2 years of experience working with older adults facing mental health and addiction issues.

Preferred Skills:

Effective interaction with high-risk, marginalized, and vulnerable populations.

Experience managing aggressive behaviors, along with strong de-escalation and boundary-setting skills.

Excellent written, verbal, and interpersonal communication abilities.

Capability to work independently and collaboratively within a team.

Proficiency in conflict resolution and crisis intervention.

Demonstrated sound judgment.

Competency in using computers, databases, and web-based applications.

Knowledge of geriatric issues, harm reduction approaches, and mental health legislation.

Familiarity with community resources and various systems in the region.

This position may require some evening and weekend shifts and may involve exposure to second-hand smoke, odors, pests, and clients' potential use of inappropriate language.
