Job Summary:
As a Community Care Specialist with the Texas Department of Aging & Disability Services, you will play a vital role in determining the needs and eligibility of individuals for Community Care services. Your primary responsibilities will include conducting home visits, interviewing individuals, and documenting information to determine the need for services and developing service plans. You will also verify data, explain program benefits and requirements, refer individuals for appropriate services, and ensure authorizations are registered in automated systems.

**Key Responsibilities:**

• Attend work on a regular and predictable schedule, adhering to agency leave policy and perform other duties as assigned.
• Conduct interviews with applicants, individuals receiving services, and resource persons, and conduct home/site visits to assess the need for services and provide information and referral to other resources.
• Conduct reviews to determine eligibility and functional needs.
• Obtain, verify, and calculate income and resources to determine financial eligibility for Title XX programs.
• Maintain case record documents and enter information in an automated system, establishing a record for each individual receiving service.
• Monitor services through contacting individuals receiving services to determine if services are meeting the individual's needs.
• Develop/coordinate/review service plans with individuals receiving services and their families, provider agencies, and other state agency staff, and authorize services appropriately and accurately to meet the individual's needs.
• Prepare basic level ongoing or special narrative or statistical reports.

**Requirements:**

• Ability to effectively communicate orally and in writing.
• Ability to set priorities, establish timeframes, and meet deadlines.
• Ability to establish and maintain effective relationships with individuals receiving services, co-workers, contract agency staff, and staff from other federal or state agencies.
• Ability to operate computer and general office equipment.
• Experience in interviewing techniques to obtain personal information, to make inquiries, and to resolve conflicting statements.
• Experience with resources that serve individuals who are older or have a disability.
