Job Description:

The Continuity of Services Specialist is a key member of the State Supported Living Center Program and Continuity team at the state office, reporting to the State Supported Living Center Program Director. This telework role requires some travel and can be based anywhere in Texas. The specialist is responsible for conducting annual internal improvement reviews of each of the thirteen SSLCs, providing highly advanced consultative services, and technical assistance in the planning, implementation, and monitoring of programs and services across the SSLCs. The specialist works closely with the State Office Continuity of Services Coordinator and Continuity of Services Specialists, Living Options Specialist, Transition Specialist Coordinator, and other discipline leads to develop and provide recommendations to SSLCs regarding service planning and implementation processes. The specialist also conducts routine monitoring of Community Living Discharge Plans (CLDPs), Potentially Disruptive Community Transitions (PDCTs), Transition Logs, the individualized services planning process, and active treatment records, and uses the information to develop recommendations for initiatives and training. The specialist designs and develops training materials and provides training to all levels of center staff to a variety of group sizes, including large groups. The specialist also collects, organizes, and analyzes information to identify trends and develop plans to resolve operational issues. The specialist conducts organizational and administrative statewide Continuity of Services related tasks, reviews and analyzes compliance reports, and develops responses with the appropriate State Office discipline lead. The specialist assists with the completion of compliance reviews and state review team visits. This role requires flexibility and highly skilled time management, including the ability to simultaneously manage high-priority projects and unexpected tasks that need immediate attention. The specialist works under minimal supervision, with considerable latitude for the use of initiative and independent judgment.

Key Responsibilities:
  • Conduct state review team visits, including on-site and remote evaluation of community discharge planning and implementation of said plans, post-move monitoring review, documentation review related to Continuity of Services, develop, and ensures compliance, identify potential improvement areas, and make recommendations for improvement for the thirteen (13) SSLC.
  • Serve as a subject matter expert for the SSLCs, providing routine organizational and administrative statewide continuity of services tasks such as maintaining the admissions and placements update, managing COS data, creating technical reporting from review, and making recommendations for improvements and ensuring compliance with policies and procedures.
  • Provide technical assistance to forensic center admissions and placements staff as well as state office discipline coordinators as needed, and provide routine monitoring of Living Options Discussions (LODs), obstacles to referral or transition, Community Living Discharge Plans (CLDPs), Post Move Monitoring (PMM), Potentially Disruptive Community Transitions (PDCTs), transition logs, the individualized services planning process, assessments, and active treatment records, and uses the information to develop recommendations for initiatives and training.
  • Plan, prioritize, and provide highly complex consultation and technical assistance and makes recommendations to the SSLCs to ensure effective and consistent program evaluation and monitoring across the SSLCs, analyze resources, and provide complex technical assistance to develop aids and tools to supplement or replace current resources.
  • Review compliance reports related to service planning and implementation and works collaboratively with the appropriate state office discipline lead to develop responses, prepare and present activity and status reports, verbally and in writing, and assist with providing support to facility Admissions and Placements coordinators and their department staff for the Department of Justice (DOJ) settlement agreement visits by providing instructional materials, offering guidance, making recommendations, analyze data and improve strategies.
