Key Responsibilities

The primary role of the Wraparound Facilitator is to provide empowerment, guidance, and care coordination for youth and families. This position involves engaging high-need youth and their families in the creation, implementation, and monitoring of a personalized care plan. Responsibilities include:

  • Facilitating Child and Family Team meetings.
  • Leading the development and evaluation of individualized plans through the Wraparound process.
  • Building trust and mutual respect with youth and families.
  • Assisting teams in identifying needs and creating a shared vision.
  • Helping families recruit and engage natural support systems.
  • Empowering youth and families to take ownership of the Wraparound process.
  • Supporting families in recognizing and leveraging their strengths.
  • Identifying barriers to participation and developing strategies to address them.
  • Ensuring adherence to the Wraparound model with high fidelity and tracking progress toward goals.
  • Providing advocacy and support for the Wraparound team.
  • Preparing documentation and reports as required.
  • Maintaining confidentiality and compliance documentation for each youth.
  • Educating families and community members about the Wraparound process.
  • Contributing to a positive, team-oriented work environment.
  • Documenting services and treatment plans in accordance with clinical and administrative policies.
  • Collaborating with Wraparound Coach/Supervisor and Family/Youth Partner to enhance team rapport and address challenges.
Qualifications

Required:

  • Bachelor's degree in human services or a related field.
  • A minimum of two years of relevant professional experience.
  • Familiarity with case management database administration is preferred.
  • Knowledge of community resources, preferably with local experience.
  • Experience working with or having lived with children with complex needs.
  • A valid driver's license and compliance with any necessary credentialing or licensing standards.
