Job Summary

We are seeking a compassionate and dedicated Care Coordinator to join our team at Anderson Behavioral Health. The successful candidate will be responsible for creating a safe and therapeutic environment for adolescents, building on client strengths, and facilitating Child and Family Team meetings.

Key Responsibilities
  • Coordinate and facilitate Child and Family Team meetings to ensure effective collaboration and communication among team members.
  • Develop, implement, and evaluate individualized services plans to meet the unique needs of each client.
  • Establish trust and mutual respect with youth, families, and team members to create a positive and supportive environment.
  • Assist the team in identifying needs and creating a shared vision for each client.
  • Recruit and engage natural supports to enhance the client's support network.
  • Enable youth and families to take ownership of the Wraparound process and identify their strengths and areas for growth.
  • Identify and develop strategies to overcome barriers to full participation in the program service process.
  • Prepare summary documents and final reports to ensure accurate and timely communication.
  • Ensure that all required confidentiality and compliance documents are current for each client.
  • Educate families, providers, and community members about the elements of the program and its processes.
  • Contribute to a positive and team-oriented work environment by promoting collaboration and respect among team members.
Requirements

We are seeking a candidate with a Bachelor's Degree in a Human Services related field and at least two years of experience providing services for children and families. The ideal candidate will possess strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

The Care Coordinator will be required to work in a variety of settings, including offices, schools, and community-based locations. The position will also require the ability to lift individuals and participate in physical interventions, if necessary.
