Job Summary

The RN Case Manager is a critical role at CVS Health, responsible for providing high-quality, patient-centered care to our members. As a RN Case Manager, you will be the primary point of contact for members, assessing their medical needs and developing personalized care plans to promote optimal health outcomes.

Key Responsibilities

  • Conduct thorough assessments of members' medical needs, including reviewing medical history, current health status, and social determinants of health.
  • Develop and implement individualized care plans that address members' unique needs and goals.
  • Collaborate with healthcare providers, community resources, and other stakeholders to ensure seamless care coordination and optimal health outcomes.
  • Monitor members' progress, making adjustments to care plans as needed to ensure effective management of chronic conditions and prevention of hospital readmissions.
  • Provide education and support to members and their caregivers to promote self-management of chronic conditions and healthy behaviors.
  • Document all interactions with members, including assessments, care plans, and progress notes, in a timely and accurate manner.

Requirements

  • Active and unrestricted RN license in the state of North Carolina.
  • 2+ years of clinical experience as an RN, with a focus on case management or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  • Ability to work in a fast-paced environment, with a high level of accuracy and attention to detail.
  • Proficiency in Microsoft Office Suite, with experience using electronic health records and other healthcare software.

Preferred Qualifications

  • Certified Case Manager (CCM) certification.
  • 3+ years of experience with case management in an integrated model.
  • Discharge planning experience.
  • Managed care experience.
  • BSN degree.
