Role Responsibilities:
As an Integrated Care Management Specialist, you will:
- Deliver comprehensive, person-centered care management in various settings, including community locations and individual residences.
- Facilitate holistic health management aimed at improving overall well-being by addressing physical, behavioral, and social health needs.
- Ensure consistent high-quality care for individuals with mental health, substance use, and developmental disabilities while adhering to organizational, state, and federal guidelines.
Compensation:
$15.00-$22.00
Qualifications:
Education: High School Diploma or equivalent; minimum age of 18 years.
Experience: Candidates must meet at least one of the following criteria:
- Personal experience with an intellectual or developmental disability or traumatic brain injury, demonstrating knowledge of the North Carolina Medicaid system.
- Experience as a Certified Peer Support Specialist with a behavioral health condition.
- As a parent or guardian of an individual with an intellectual or developmental disability or behavioral health condition, possessing at least two years of caregiving experience and familiarity with the Medicaid system.
- A minimum of two years of professional experience in outreach, engagement, and service coordination for the Tailored Care Management population.
Licensing:
Valid Driver's License required.

Conduct outreach, engage with members, coordinate services, and participate in case conferences to support care management efforts.
Collaboration:
Work collaboratively with Integrated Care Team members and community organizations to enhance service delivery.
Person-Centered Care:
Adopt a person-centered approach that emphasizes individual strengths and preferences.
Data Utilization:
Employ data strategically to ensure effective and coordinated care management services.
Safety and Compliance:
Adhere to health and safety policies while maintaining ethical standards in line with the RHA Code of Conduct.
