LPN Care Coordinator

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at MAB Community Services. As a LPN Care Coordinator, you will play a vital role in providing high-quality care and support to adults with acquired brain injuries in our small community residential programs.
Key Responsibilities:
- Provide comprehensive care and support to participants, including wound care, medication administration, and health care coordination.
- Assist in the development and implementation of health care standards, policies, and procedures.
- Review incident reports and ensure a safe and healthy environment for participants.
- Oversee the MAP Program at the residence, conducting routine audits and providing responses to medication errors.
- Conduct reviews of health care documentation to ensure program compliance.
- Develop and conduct in-service training for Adult Disability Service employees.
- Identify and facilitate the utilization of community resources.
- Provide client-specific consultation to program staff and health care providers.
- Transport participants to medical appointments and provide advocacy.
Requirements:
- Current LPN License
- Valid US driver's license for at least one year
- Ability to pass a CORI, reference, multi-state background, and driving record check
