Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. As a key member of our clinical team, you will conduct comprehensive health assessments in the comfort of patients' homes, providing unmatched insight into their overall health and well-being.

About the Role:

  • Conduct home visits to assess patients' current health, medical history, medication adherence, and social environment.
  • Develop and implement personalized care plans to address identified health risks and improve quality of care.
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to patients and their families to promote healthy behaviors and disease management.

Requirements:

  • Master's degree in Nursing or Physician Assistant studies.
  • Current RN and NP/PA licensure in the state of practice, with prescription authority.
  • Board certification in a Matrix-approved specialty.
  • Minimum 1 year experience as a Nurse Practitioner or Physician Assistant.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills.
