At Metropolitan Center for Independent Living, we are seeking a dedicated Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing client-centered case management services to individuals with disabilities and their families.

Key Responsibilities:

  • Coordinate client intakes, discharges, team meetings, and services within set parameters and timelines.
  • Identify and explore service options to meet clients' assessed needs and make referrals to outside resources based on assessment, current client need, and individualized plan.
  • Evaluate and monitor services identified in the service plan.

Requirements:

  • Bachelor's Degree from an accredited four-year college with a degree in social work, psychology, sociology, or closely-related field.
  • 1-3 years of experience as a social worker in a public or private social service agency.
  • Valid Minnesota Driver's License with acceptable and clean driving record required.

Salary: $60,000 - $80,000 per year, depending on experience.
