  • Coordinate services and collaborate with the interdisciplinary care team to support clients in achieving their goals.

  • Advocate on behalf of clients to ensure their rights and privacy are protected.
Requirements
  • Social work license or registered nurse licensure required.
  • One year case management experience in community-based services preferred.
