Job Summary: The Adult Services Coordinator at The Phoenix Residence is responsible for coordinating and delivering high-quality support services to adults with disabilities.

Salary Range: $52,000 - $62,000 per year

Duties and Responsibilities: As a key member of our team, your primary responsibilities will include:

  • Coordinating and facilitating individualized support plans to meet the unique needs of each client
  • Developing and maintaining positive relationships with clients, family members, and caregivers
  • Collaborating with healthcare professionals and other stakeholders to ensure seamless care coordination
  • Monitoring and evaluating client progress, making adjustments to support plans as needed

Requirements: To excel in this role, you must possess:

  • A high school diploma or equivalent
  • At least one year of experience working with individuals with physical and/or developmental disabilities
  • Excellent communication and interpersonal skills
  • Able to lift up to 50 pounds and perform tasks that require standing, walking, and bending
