Job Title: Physical Therapist (Home Health)

LHC Group is seeking a skilled Physical Therapist to join our team in Meridian, Mississippi.

About the Role:

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with LHC Group, you will have the opportunity to work with a diverse range of patients and develop a personalized treatment plan to meet their unique needs.

Responsibilities:
  • Evaluate patient care needs and develop a treatment plan to improve functional status, mobility, and overall well-being.
  • Perform interventions to enhance patient outcomes and provide education to patients and their families.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Document patient progress and outcomes to inform treatment decisions and quality improvement initiatives.
Requirements:
  • Current Physical Therapy licensure in the state of Mississippi.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
