**About the Role**

We're seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Home Health Physical Therapist, you'll work with patients in their homes, providing one-on-one therapy sessions to help them recover from illness or injury. You'll work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.

**Responsibilities**

* Provide physical therapy services to patients in their homes, including evaluations, treatments, and progress assessments
* Develop and implement individualized care plans that meet the unique needs of each patient
* Collaborate with our interdisciplinary team to ensure seamless care coordination
* Communicate effectively with patients, families, and healthcare providers to ensure optimal care outcomes

**Requirements**

* Master's degree in Physical Therapy from an accredited program
* Current licensure to practice as a Physical Therapist in the state of operation
* Valid CPR certification
* Ability to travel to all business locations
