We are seeking a skilled Physical Therapist to join our Home Health team at St. Luke's Health System. As a Physical Therapist, you will play a critical role in providing exceptional patient-centered care to our patients in the comfort of their own homes.

Key Responsibilities
  • Develop and implement individualized rehabilitation plans to help patients achieve their goals and improve their quality of life.
  • Conduct thorough assessments and evaluations to identify patients' needs and develop effective treatment plans.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Provide education and training to patients and their families on proper techniques and strategies for managing their conditions.
  • Document patient progress and outcomes in a timely and accurate manner.
