At Trinity Health Of New England, we are seeking a dedicated and compassionate Medical Care Coordinator to join our team in coordinating patient care, assessing needs, and developing care plans. As an RN Inpatient Care Coordinator, you will collaborate with healthcare providers, monitor patient progress, and ensure a smooth transition between different stages of care while advocating for the patients' well-being.

Job Summary

In this critical role, you will assist in creating and managing nursing care plans, participating in the evaluation of current and future medical needs of the patient. With a strong focus on providing exceptional patient care, you will work closely with the healthcare team to deliver high-quality services that meet the unique needs of each patient.

This position requires a Registered Nurse (RN) with a minimum of three years of diversified experience in an acute care facility or public health nurse setting. The ideal candidate will have a solid understanding of managed care methodologies, HCFA/OBRA, and state regulatory compliance issues, as well as JCAHO accreditation accountabilities of the Continuum of Care.

Key Responsibilities
  • Coordinate patient care, assess needs, and develop care plans in collaboration with healthcare providers.
  • Monitor patient progress and ensure a smooth transition between different stages of care.
  • Assist in creating and managing nursing care plans.
  • Participate in the evaluation of current and future medical needs of the patient.
Requirements
  • Bachelor's degree in Nursing (BSN) from an accredited college or university.
  • Current licensure as a Registered Nurse (RN) in Connecticut.
  • Certification in Case Management (ACM, CCM) required within one year of employment.
  • Minimum of three years of experience in an acute care facility or public health nurse setting.
Preferred Qualifications
  • Experience working with nationally recognized medical utilization criteria sets (i.e. MCG, InterQual).
  • Solid understanding of managed care methodologies, HCFA/OBRA, and state regulatory compliance issues.
  • JCAHO accreditation accountabilities of the Continuum of Care.
Salary Information

The estimated annual salary for this position is $85,000-$110,000, depending on experience and qualifications.
