Patient Care Coordinator

We are seeking an exceptional Patient Care Coordinator - Discharge Planning to join our team at Della Infotech, Inc. As a vital member of our interdisciplinary team, you will be responsible for coordinating the discharge process for patients, ensuring timely and safe transitions to home or alternate levels of care.
Your key responsibilities will include conducting comprehensive assessments to identify patient needs, developing and implementing individualized care plans, and coordinating care with multidisciplinary teams. You will also communicate with patients, families, and healthcare providers to ensure seamless transitions and provide education on self-care and community resources.
In this role, you will work collaboratively with internal and external disciplines to optimize patient outcomes, minimize readmissions, and enhance the overall patient experience. We offer a competitive estimated salary range of $60,000 - $85,000 per year, commensurate with experience.
