Patient Care Coordinator

We are seeking a highly skilled Patient Care Coordinator to join our team in the Thoracic Specialty department. As a critical member of our healthcare team, you will play a vital role in creating and managing nursing care plans, while collaborating with patients, families, and medical professionals to deliver exceptional patient-centered care.
Key Responsibilities- Act as the single point of contact for thoracic patients, providing seamless communication and coordination of care
- Develop and implement comprehensive nursing care plans in collaboration with multidisciplinary teams
- Promote evidence-based practice and research within clinical settings
- Graduate of an accredited School of Nursing with a Bachelor's degree in nursing or Associate's degree in nursing with at least five years of nursing experience
- Current licensure as a registered nurse (RN) in Massachusetts
- CPR certification
- Three to five years of nursing experience
