Community Care Nurse, Full Time

We are seeking a highly skilled and compassionate Registered Nurse to join our Community Care team at Rhode Island Hospital. As a key member of our healthcare team, you will provide exceptional patient care and support to our patients and their families.
Key Responsibilities- Implement, supervise, and evaluate the nursing care of each patient using a nursing process and in accordance with established hospital and departmental policies and procedures.
- Plan and provide direct patient care, taking into account the aging process and cultural patterns throughout the life span.
- Provide leadership by delegating, supervising, and participating in safe nursing care activities provided by ancillary personnel.
- Evaluate the effectiveness of patient care and identify opportunities for improvement.
- Collaborate with the healthcare team, patient, family, and community services regarding patient care outcomes and discharge planning.
- Assess and provide appropriate teaching based on patient and family needs.
- Participate in unit-based quality improvement and research activities.
- Utilize all available resources to provide quality, cost-effective patient care.
- Licensure as a Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
- Baccalaureate in Nursing preferred.
- Current licensure to practice as a Registered Nurse in the State of Rhode Island.
- Demonstrates knowledge and skills necessary to provide care to patients throughout the life span, considering aging processes, human development stages, and cultural patterns in each step of the care process.
- Visual and aural ability to monitor patient status.
- Physical ability to assist in lifting and positioning patients.
- Exposed to patient care environment.
- Physical ability for extensive walking and standing; lifting, positioning, and pushing patients on stretchers.
